Who is applying

Identity

Federal law requires SSN on the SSI application — there is no opt-out path. Default: leave blank here and bring your SSN card to the SSA interview. Tap 'Enter digitally' below to encrypt your SSN on our servers for a fully-completed PDF (decrypted only in-memory during render; auto-deletes 30 days after submission).

Marital + household

Same rule as your own SSN — default skip, tap to enter digitally with encryption.

Living arrangement

SSI payment amount depends on whether you live alone, with others, in an institution, or are homeless. Be specific.

If you're escaping abuse or unsafe at your current address, ask the partner where you'll submit this form whether you can use a safe-address (FJC, library, or partner agency). We won't track which address you used.

How we handle addresses →

SSA counts in-kind support against your benefit — be honest, lying voids the application.

Income — all sources

List each source and monthly amount.

Resources (assets you own)

SSI has hard resource limits: $2,000 individual / $3,000 couple. Some things don't count: your primary home, one vehicle, household goods. Be complete — the SSA cross-checks bank records.

One vehicle is excluded regardless of value; additional vehicles count.

Policies with total face value over $1,500 may count as a resource.
