Identifying information

If you were born outside California, the birth certificate request goes to that state, not CDPH.

Needed for the homeless-services-provider affidavit. Describe honestly — outside, in a vehicle, in shelter, couch-surfing.

Provider affidavit — to be signed in person by Guardian navigator
